I looked pretty closely at the field last August, but I won't claim these are necessarily "the best". This is a fast-evolving space with lots of vendors. There are some big players with high-end data management capabilities that let you do a LOT of really powerful things with the data you collect, i.e. a lot more than forms management. One I've had experience with is Accela. They aren't cheap, but their product is exceedingly sophisticated in terms of what you can do with the data you collect.

If cost is a concern and the volume of data to be collected isn't enormous, take a look at WuFoo. Very user-friendly, quite powerful and configurable. If the main focus is just deploying good web forms quickly and easily, these guys are worth a look.
